PT-141 (bremelanotide) is a synthetic melanocortin receptor agonist that works on the central nervous system — not on blood flow like Viagra. By activating melanocortin receptors (MC3R and MC4R) in the hypothalamus, PT-141 triggers the neural pathways involved in sexual desire and arousal. Unlike hormonal treatments or vascular medications, it acts on the brain’s desire circuitry itself. The same active ingredient is FDA-approved under the brand name Vyleesi® for acquired, generalized hypoactive sexual desire disorder (HSDD) in premenopausal women.

PT-141 is a subcutaneous injection you give yourself at home, on-demand, at least 45 minutes before anticipated sexual activity. Effects typically last 8–10 hours. Do not use more than once in 24 hours and no more than 8 doses per month — this is a firm safety limit, not a suggestion. It is not taken daily. Your first shipment includes the medication, syringes, alcohol swabs, and a step-by-step guide.

Many women report increased sexual desire, stronger arousal response, and a general reconnection with their libido. Results vary — some women feel a noticeable effect from the first dose, others need 2–3 doses to evaluate response. PT-141 does not create desire where there is none; it amplifies existing pathways. If you have not responded after 8 appropriately-timed doses, discuss next steps with your provider. Results may vary.

The most common side effects are nausea (especially the first few doses — anti-nausea strategies can help), flushing, injection site reactions, and headache. Two important warnings: (1) PT-141 causes a transient increase in blood pressure and decrease in heart rate for several hours after dosing — it is not appropriate for patients with uncontrolled hypertension or significant cardiovascular disease. (2) Repeated use can cause focal skin and gum hyperpigmentation (darkening), especially with more frequent dosing, which is why the 8-doses-per-month limit exists.

Do not use if you have uncontrolled hypertension or established cardiovascular disease, if you’re pregnant or planning pregnancy (PT-141 is contraindicated during pregnancy — use effective contraception), or if you have a known hypersensitivity to bremelanotide. PT-141 significantly decreases the absorption of oral naltrexone — patients taking naltrexone for alcohol or opioid use disorder should not use PT-141. Always disclose your full medication list and history during intake.

The active ingredient, bremelanotide, is FDA-approved as Vyleesi® (AMAG Pharmaceuticals) for premenopausal women with acquired, generalized HSDD. Our compounded PT-141 contains the same active ingredient but is prepared by U.S. state-licensed compounding pharmacies and is not individually FDA-approved. Compounded PT-141 is legal when prescribed by a licensed provider and offers a more affordable, accessible alternative for patients whose insurance doesn’t cover Vyleesi®. Your provider will determine if it’s clinically appropriate.
